Required Documents for Membership

• UNIW Membership Application Form (Must be filled out digitally)

• Board of Directors decision declaring your organization's desire to join UNIW and assigning the Representative to UNIW. The decision must be written in your organization's decision book and signed by all board members.

• Signing and sending the last page of the UNIW charter to UNIW with the notation "read, understood, and accept" by the authorized signatory/signatories of the legal entity.

• Every page of the charter signed by the authorized signatory/signatories of the legal entity.

• Two Recommendation Letters from existing UNIW Member Organizations.

• Activity Report of your organization (if available).

• Official Registry/Certificate of Operation showing the organization is registered with the state.

• Vector and JPEG versions of your organization's logo.

• One-year membership fee (Can be paid in cash or via bank transfer).

Note: The annual membership fee is $1200 (One Thousand Two Hundred USD). This amount must be paid during the application.
